Apuntes de interés: 1- los coronavirus pueden mantenerse durante mucho tiempo en las superficies (de 2 h a 9 días). Depende de variables tales como: a) el inóculo: cuanto mayor la cantidad de viriones, mayor la duración en la superficie; b) la temperatura: el calor disminuye la duración, y el frío la aumenta (¡atención neveras! – a 4ºC puede durar más de 28 días), c) la humedad (cuanta más humedad, mayor duración); y d) y otras (tipo de superficie, el tipo específico de coronavirus …) 2- el efecto de los distintos agentes biocidas suele ser razonablemente bueno. Entre los que nos interesan: a) la lejía (hipoclorito sódico) al 0.1% durante 1 min tiene alta tasa de erradicación (atención, muchas lejías vienen como disolución al 5% –> hay que hacer una disolución 1:50 y no la estándar de 1:100); b) las soluciones con etanol 62-71% (similares a las que usamos en el hospital) durante 1 min también tiene buena tasa de erradicación. RESUMEN: Estudio retrospectivo y multicéntrico (2 hospitales) que describe una cohorte de pacientes (n=191) con diagnostico microbiológicamente confirmado de infección por SARS-CoV-2 y outcome clínico definido (muerte intra-hospitalaria vs. alta hospitalaria), buscando identificar factores asociados con mortalidad. Murieron 54 pacientes (28%). El tiempo mediano desde el inicio de la disnea hasta la intubación en los pacientes que recibieron ventilación mecánica fue de 10 días (RIC 5.0-12.5). La duración mediana de aclaramiento viral en supervivientes fue de 20 días (RIC 17-24) y en no-supervivientes fue continua hasta su fallecimiento. Los factores asociados con mortalidad intra-hospitalaria en el análisis univariante fueron la edad, comorbilidad (cardiopatía isquémica, diabetes, HTA), linfopenia, leucocitosis y elevación de GPT, LDH, Hs-Tnt, CK, D-dímero, ferritina sérica, IL-6, creatinina, tiempo de protrombina y procalcitonina. Entre las variables incluidas en el análisis multivariante, se identificaron como factores independientes asociados a mortalidad una mayor edad, una mayor puntuación del SOFA score (que valora la disfunción orgánica) y niveles de D-dímeros por encima de 1µg/L. RESUMEN: Los diferentes miembros de la familia lancetcoronavirus responsables del MERS-CoV y el SARS-CoV fueron responsables de complicaciones durante el embarazo. Entre 2002 y 2003, 12 mujeres se infectaron con SARS-CoV, cuatro pacientes infectadas en el primer trimestre presentaron un aborto.  En el segundo al tercer trimestre, dos (40%) de cinco mujeres tuvieron crecimiento intrauterino retardado y cuatro (80%) de las cinco mujeres tuvieron un parto prematuro (uno espontáneo; tres inducidos por afección materna). Tres (25%) mujeres murieron durante el embarazo. En una revisión de 11 mujeres embarazadas infectadas con MERS-CoV, 3 diez (91%) presentaron resultados adversos, seis (55%) neonatos requirieron ingreso a la unidad de cuidados intensivos y tres (27%) fallecieron. Dos nacieron  prematuramente por insuficiencia respiratoria materna severa.
